Outpatient Substance Use Disorder Treatment
The Family Health & Wellness Center provides outpatient substance use disorder treatment for individuals seeking support with substance use, recovery, and long-term wellness. Services are culturally responsive, trauma-informed, and individualized to each person’s needs, strengths, and stage of recovery.
Our team understands that substance use can affect many areas of life, including physical and emotional health, family relationships, school or work, culture, and community. Treatment is designed to address the whole person while helping individuals build practical skills, strengthen protective factors, and work toward meaningful recovery goals.
Programs
- Outpatient Treatment – ASAM Level 1.0
Structured outpatient services for individuals who can safely participate in treatment while continuing to live at home and remain engaged in their daily responsibilities. - Intensive Outpatient Treatment – ASAM Level 2.1
A more structured level of outpatient care for individuals who need additional therapeutic support, accountability, and recovery-focused services without requiring residential treatment.

- Substance Use Disorder Assessments
Comprehensive assessments help identify substance use patterns, strengths, risks, treatment needs, and the most appropriate level of care. - Individual Counseling
One-on-one support focused on recovery goals, behavior change, coping skills, motivation, and the factors that may contribute to substance use. - Group Counseling and Recovery Support
Structured group services provide opportunities to build skills, strengthen recovery, increase peer support, and learn from shared experiences. - Individualized Treatment and Recovery Planning
Each person participates in developing a treatment plan based on their needs, goals, strengths, culture, and recovery priorities. - Relapse Prevention and Recovery Skills
Treatment helps individuals identify triggers, strengthen coping strategies, develop healthy routines, and build skills that support long-term recovery. - Family Support and Education
When appropriate, families and caregivers may be involved to strengthen communication, increase understanding of substance use disorder, and support recovery at home. - Coordinated Care
Substance use disorder services may be coordinated with mental health, primary care, and other providers to support a more connected approach to health and recovery. - Referrals and Care Navigation
When additional services or a different level of care is needed, our team can help connect individuals with appropriate treatment, community resources, and support services.
